Discharge Coordinator

The Discharge Coordinator is responsible for performing accurate, timely and effective discharge planning for all patients. Promotes safe and continuous post hospital care.



Job Responsibilities/Duties

  • Ability to communicate to patients, physicians, visitors, and employees in a concise and effective manner.
  • Ability to apply common ­sense principles, along with knowledge base to analyze and solve problems that arise.
  • Ability to define a complex problem, collect data, establish data, and draw a valid conclusion.
  • Attention to detail and strong  organizational skills. Ability to prioritize tasks and ability to learn quickly in a busy office.
  • Proficiency in Microsoft Word and Excel, and Internet research skills.
  • Must exhibit excellent customer service and telephone skills. Must enjoy helping seniors.
  • Maintain Confidentiality of sensitive matters and documents.  


Qualifications

Minimum Education: A minimum of a high school diploma or equivalent. Bachelor's degree preferred, College courses in business administration and/or healthcare administration preferred.              

Minimum Experience: Nursing home care experience or experience in a multi-disciplinary clinic setting preferred, one (1) year specialty area experience preferred.

Req. Certification/Licensure: None.
