Specialist - Hospital Care Coordination

The Hospital Care Coordination Specialist plans, implements, and oversees programs designed to advance the objectives of the Hospital Care Coordination Department.   Assists with a variety of projects, reports, health fairs, internal and external events, and other tasks that support the goals and objectives of the department.  Assists with hospital programs and services, including Intensive Outpatient Programs (IOP) and Psychiatric Programs.



Job Responsibilities/Duties

  • Identifies, visits, and provides information about hospital program/services to extended care facilities that are within hospital service area.  Demonstrates complete understanding of the programs/services that our hospitals offer.  Keeps accurate records for follow up and outreach.
  • Oversees the efforts of the community education team to ensure all areas are properly covered.  Reports community information and status updates of issues/resolutions/results to the Director of Hospital Care Coordination.  Maintains accurate records of activities/events held by Community Outreach staff.  Follows up to ensure customer satisfaction. 
  • Assists the Director, Hospital Care Coordination on a variety of projects, reports, health fairs, and other tasks that support the goals/objectives of department.  Assists with internal and external events and programs designed to enhance awareness of hospital services to the community.
  • Completes reports as appropriate (or requested) on activity and progress toward meeting the goals of the department. 
  • Exhibits professionalism when dealing with others and is able to maintain constructive working relationships with co-workers and physicians.  Relates well with others including supervisors, colleagues, and internal/external customers.  Respects the rights, privacy, and property of others, and maintains strict confidentiality. 


Qualifications

Minimum Education: Bachelor’s Degree and three (3) years’ previous experience in an external customer facing environment, such as sales or Equivalent – high school with seven (7) years’ experience in an external customer facing environment.

Minimum Experience: Excellent written and verbal communication skills in English. Ability to communicate in a concise and effective manner. Ability to maintain a work pace appropriate to the workload. Ability to work independently with minimal supervision and direction. Must demonstrate customer service skills appropriate to the job. Demonstrates excellent organizational skills/abilities. Ability to establish and maintain effective working relationships. Previous experience working in healthcare program development in Los Angeles or Orange County market preferred.

Req. Certification/Licensure: Valid California Driver’s License and reliable/insured vehicle required.
