Social Worker

The Social Worker is responsible for Providing assistance to members and their families in the form of counseling, referrals to community resources. Will interpret psychosocial implications of illness to attending Providers and staff as needed.  Maintaining a minimum caseload of 40-60 cases. Capable of being independent with decision-making within the Scope of Practice and Job Description. 



Job Responsibilities/Duties

  • Develops individual treatment plans and makes recommendation for members.  Implements treatment plan using appropriate modalities, including individual counseling, medication monitoring, crisis intervention, community contacts, diversion and triage.  This can be achieved by meeting with the member and/or responsibly party in the clinic, acute/SNF, or home setting.  
  • Attend and contributes to Team Conferences regarding care to/for specific member. 
  • Interprets psychosocial implications of illness to attending staff and departmental staff as needed. Will attend and participate in Department meetings. Will consult upon request of clinical and/or educational staff members or accept referral from Case Management or P.C.P. office, for members needing resource information.
  • Prepares reports and compiles statistical data regarding members, as determined by the management team.  Promotes Basic Needs Assessment; clothing, housing/homeless prevention, limited financial resources, home maintenance repair, referrals, food, safety in the home, transportation, and utilities assistance. 
  • Health: assist with application for MediCal, insurance enhancements, prescription referral source, education on how to obtain medical equipment, Advance Directives/Hospice choices, dental care referral source, non-compliant issues, and psychiatric condition interfering with medication regiment.
  • Family/Social Support; limited social support, parenting/educational skills, domestic violence, elder care, parent-child conflict, family relations issues, recreational activities, and information regarding Respite Care for caregivers.  
  • Will provide evaluations (when requested) for all PMG members referred by High Intensity Case Managers, Concurrent review case managers, Medical Directors, PCPs or any other members of the Medical Management team, regarding possible placement, member financial concerns and/or ADL assistance needs.


Qualifications

Minimum Education:   Bachelor’s Degree in Social Work (BSW), MSW preferred

Minimum Experience:   Minimum one (1) year experience in a hospital setting. Knowledge of CMS, Medicare/Medi-Cal. Familiarity of Joint Commission, CMS, CDPH requirements. Excellent written and verbal communication skills in English. Ability to establish and maintain effective working relationships across the organization. Ability to facilitate and lead interdisciplinary rounds
