Pharmacist – Infectious Disease

The Infectious Disease Pharmacist(ID Pharmacist) is responsible for oversight, strategic planning, and optimization of antimicrobial stewardship program (ASP) throughout the Southern California Hospital system. The Infectious Disease pharmacist will perform daily monitoring of antibiotics to improve patient outcomes by promoting appropriate usage and minimizing adverse drug events, antimicrobial resistance and costs. The ID pharmacist will serve as the expert clinical resource for the pharmacy staff and the health care professionals within the organization. ID pharmacist will be responsible for developing and implementing evidence-based guidelines and protocols. ID pharmacist will perform antimicrobial formulary review, medication use evaluations and quarterly antimicrobial stewardship reports for ASP committee and P&T committee. The ID pharmacist collaborate with the system Clinical Pharmacy Specialist, Infectious Diseases in developing system initiatives.

Job Responsibilities/Duties

  • Monitors antimicrobial therapy regimens for contraindications, drug-drug interactions, drug-food interactions, adverse drug reactions, and appropriateness of drug and dose. Identifies and intervenes when problems involving drug therapy arise (i.e. inappropriate selection of antimicrobials, allergies, drug interactions, therapeutic duplication, and adverse drug reactions).
  • Documents clinical interventions in a timely and professional manner.
  • Collect and analyze antimicrobial stewardship data to generate comprehensive reports for the ASP and P&T committees.
  • Serves as a pharmacy representative of the ASP committee.
  • Works with the ASP physician and system ID pharmacist to design  and implement protocols,  policies/Procedures and order sets to improve the ASP program.
  • Monitors new processes and policies over time to ensure compliance.
  • Assist in adding to and maintaining the antimicrobial formulary, monitoring compliance over time and refining processes / policies to ensure effectiveness.
  • Facilitate, coordinate, and/ or conduct ASP education for physicians, prescribers, Infection Prevention, microbiology, nursing, residents, pharmacists, and other health care professionals.
  • Precepts pharmacy students for Antimicrobial Stewardship Program


Minimum Education: Graduate of an accredited School of Pharmacy recognized by the California State Board of Pharmacy required. Doctor of Pharmacy degree required. Completion of a generalized residency in Pharmacy Practice (PGY-1) is required. Completion of PGY-2 training in Infectious Diseases required.

Minimum Experience: One (1) year of related work experience required. Ability to work collaboratively in a multidisciplinary environment. Ability to read, write and communicate effectively in English. Strong time management skills and ability to effectively prioritize tasks. Ability to establish and maintain effective working relationships within the organization.

Req. Certification/Licensure: Basic Life Support certification & Advanced Cardiac Life Support certification; required. Licensed by the CA State Board of Pharmacy required. Board Certified Infectious Diseases Pharmacist® (BCIDP), SIDP Antimicrobial Stewardship Certification, or similar Infectious Disease or Antimicrobial Stewardship certification preferred.
