The Lead Case Manager BHU UR is professional nurse who will coordinate the duties of the Case Managers in their timely completion of assessments for appropriateness of admission and continuation of stay and to prepare the patient for discharge. The Lead Case Manager will oversee documentation of administrative days and monitor the Case Managers to ensure completion of routine documentation in a timely manner, oversee department’s needs and submit findings to the Director, coordinates initiation of departmental contacts with a variety of disciplines, maintain contact with providers, arrange peer reviews with MDs. The Lead Case Manager is responsible for the effectiveness and efficiency of the department, by supervision and evaluation of the department outcomes.

Job Responsibilities/Duties

  • Coordinates, initiates and oversees documentation of admission review the first working day after admission.
  • Monitors and ensures documentation of assessment of appropriateness of admission and continuation of staff.
  • Oversees the documentation of Administrative Days.
  • Coordinates initiation of departmental interdisciplinary contacts.
  • Supervises contacts with providers.
  • Arrange timely peer reviews with MD.
  • Integrates department services with other departments to assure coordinated hospital operations.
  • Communicates effectively in writing and verbally to ensure understanding, leads through listening, questioning, etc.
  • Effectively leads his/her department through change dealing constructively with resistance to change.
  • Effective in team building and negotiation.


Minimum Education: High School Diploma.

Minimum Experience: Minimum of three (3) years experience in an acute hospital setting. Expertise in one of the following areas: Health care management, Quality assessment and improvement, Continuous quality improvement, Utilization review, Discharge planning, Social service, Infection control, Risk management, Financial management, Health information management, Clinical pathway development. Evidence of skills in research, communications (both written and oral), organization, creativity, leadership, group facilitation, and time management. Ability to relate to and maintain a rapport with all levels of professional personnel. Computer experience and knowledge in the management of computer data desirable.

Req. Certification/Licensure: LVN or RN license.
