Behavioral Health Counselor - Per Diem

The Behavioral Health Counselor is responsible for facilitating initial and ongoing assessments. Conducts individual and group sessions relating to the treatment needs of the psychiatric patient. Functions as a member of the interdisciplinary treatment team to coordinate care objectives.



Job Responsibilities/Duties

  • Completes initial interviews and psychosocial history with patients in a concise, legible and timely manner.
  • Develops, documents, review and updates all individual patient treatment plans and charts all pertinent information.
  • Identifies appropriate treatment intervention utilizing clinical methodology.
  • Coordinates the individualized treatment goals with the interdisciplinary team. Documents interventions on the Interdisciplinary Treatment Plan form.
  • Assists the patient to establish a discharge plan for continued support of identified goals as well as involvement in leisure and social outlets.


Qualifications

Minimum Education: Group Therapist training: MFT, PhD, PsyD, LCSW.

Minimum Experience: Three (3) years experience in psychiatric rehabilitation setting, outpatient preferred. Case management, group therapy, and individual therapy experience.

Req. Certification/Licensure: Current licensure as MFT, PhD, PsyD, LCSW. American Heart Association (BLS) Certification.
